July 21 – School Athletic Physicals
5:00-8:00pm at the Shaw Medical Building, 1111 Kentucky Avenue. OMC will offer free athletic physicals to student athletes. All students will be required to submit a signed consent form and current insurance information from a parent or legal guardian. Consent forms may be picked up at West Plains High School in the downstairs office, at No. 3 Parkway Shopping Center, or at OMC Rehabilitation Services located on the second floor of the Shaw Medical Building at 1111 Kentucky Avenue.
Students should arrive according to the first letter of their last name.
5:00-5:30pm – A to E
5:30-6:00pm – F to I
6:00-6:30pm – J to M
6:30-7:00pm – N to Q
7:00-7:30pm – R to U
7:30-8:00pm – V to Z
